Wmo: Social Support Act

Appropriate respite care is determined per person.

Respite care is sometimes free, for example because it is voluntary work. If it does involve costs, the informal carer may be able to be reimbursed. This is possible if the carer himself receives support from the Wmo. In that case, the informal carer can apply for respite care at the municipality.

For applications, support and information on the Social Support Act and for the application PGB for WMO support, please contact your municipality through the Social Neighbourhood Team in your area. Lelystad has four social district teams (North West, North East, South West and South East) and you can call them at the general telephone number 0320-745720. You can also e-mail: contact@wijkteamlelystad.nl

The staff of the Social District Team know the opportunities available in Lelystad.

For more information, you can also contact the Mantelzorg Steunpunt.

WLZ: Long-term Care Act

Lodge accommodation for people who need permanent (medical) supervision.

If the care recipient receives care from the Long-Term Care Act (Wlz) via a personal budget (pgb), respite care can also be purchased from the same budget for a maximum of 156 24-hour days per year (an average of 2 24-hour days per week). The care office must approve the care description and care contract for the requested pgb respite care. It concerns a client living at home with CIZ (Centrum Indicatiestelling Zorg) indication.

PGB - WLZ

A personal budget (pgb) under the Long-Term Care Act (Wlz) allows you to arrange help and support yourself. Only after a particular help has been granted can you choose either care in kind (ZIN) or a pgb (personal budget):

  • You apply for a Wlz indication at the CIZ - 088-789 10 00.
  • You apply for a pgb-Wlz at the care office. Zorgkantoor Flevoland (Municipalities: Dronten, Lelystad, Noordoostpolder, Urk, Zeewolde) Zwolle -038 456 78 88

Be well informed about the pros and cons of a pgb.

Applying for a Wlz indication

Do you need care from the Long-Term Care Act (Wlz)? For example, because you are moving to an institution? If so, you can apply for an indication for this. Your indication indicates the type and form of care you will receive. The Care Assessment Centre (CIZ) needs a lot of information about your personal situation, your health and your living conditions. You must fill in these details on the application form.

You can download the application from My Wlz Application. You can also download the form from the CIZ website and send it by post.

Youth Act

Under the Youth Act, children with a disability, disorder or condition may be eligible for lodging care, care farm or host family. An application can be made to the municipality for this. But also day care (medical day care) and/or support at home.

Care in kind (Zin)

You can also opt for care in kind. Lelystad municipality has made agreements with several providers. You will then receive the support you need through one of these main providers. You then have less administrative tasks to deal with; the municipality does that for you. You can therefore choose whether you want care in kind or a PGB.

Personal budget (PGB), what does it mean?

A personal budget (PGB) is an amount you can use to buy support for yourself.

A combination of PGB and ZIN

It is also possible to purchase part of the support with a PGB and receive another part as care in kind.

Want to know more about Zin or the PGB and whether you are eligible? If so, please contact Youth Lelystad (JEL) at: 0320-215144 (Mon to Fri from 09:00 to 17:00) or info@jeugdlelystad.nl.
